Free iTraining: Adolescent Care Management

On the third Thursday of each month, the Addiction Technology Transfer Center (ATTC) hosts free iTrainings surrounding various topics affecting the addictions treatment and recovery services fields. This month's iTraining is "Adolescent Care Management--An Emerging Health Care Reform Priority".

Under the Affordable Care Act, all health care providers including substance use disorder specialists are charged with improving program and client outcomes. To meet this objective, providers must become adept at a concept called Care Management.
The American healthcare system dedicates considerable resources to the acute post-care phase, but leave the client to develop their own program of recovery. As a result, outcomes too often suffer. The Care Management program utilizes trained Care Coordinators who partner with youth, families and community providers to strengthen linkages that will lead to improved health outcomes. The approach centers around motivational interviewing, client driven service plans, care conferences and understanding a Recovery Oriented System of Care.
This training will:
-expose participants to research related to the Care Coordination model
-provide a framework for High Performing Care Coordinators
-Define the Care Coordination Competencies
-Define the role of a Care Coordinator
-Review key practices
-Provide an understanding of service domains and examples of one county already successfully implementing this model
